Gamefreak didn't mess up - Venomoth just happens to be based off of a butterfly too. The organ on Caterpie's head is a osmeterium, which are found on the Swallowtail species of Butterfly that Caterpie is based off of. Butterfree's wings look more like this though.

Not really a secret, more like a Skyward Sword reference. I checked the original design of the harp/lire in the original OoT and the intro to Melee and it's design has indeed been altered since then (in the old games it was more balled at the end, now it's more hooked, like the one Zelda has in SS).

I'm also pretty sure that Zelda from Skyward Sword is based upon an old design of her, like LttP or Zelda II old.

There are also speculations that the bird on the Hylian shield is actually based upon the birds that Link and his people rides in Skyward Sword
