Playing against Abel in Street Fighter 4. Bet you didn't expect a fighting game on there, but seriously, every time I fight him, whether it be human players or the computer on any difficulty, I have ENORMOUS problems.
Most other annoying levels I think I end up repressing, that's why I can't remember them all. However, I consistently hate levels with annoying flying enemies, either the kind that home in on you, but in fast, annoying ways that avoid them getting hit, or enemies that have annoying flying patterns.

Gradius's last stage.
its not that the whole game gets incredibly hard with the last level being a total punch to the balls, its that all that shit just to kill an excuse of a final boss.
and while we're at it, Robocop Vs Terminator's last boss on the sega Genesis. great, hard as shit game, completely monotonous/unfair/gay as hell/stupid last boss that can only be killed with 1 weapon obtainable on a secret level and from a mid-game boss(ED-209) and racking more than 13 lives.

Most annoying levels? Here are some of 'em from Super Mario Sunshine:
1) On Isle Delfino, taking Yoshi across the water without drowning him or starving him to unlock a special course which is...
2) ...An eight red coin special course set on a poison water river that you drive a leaf boat down. Touching the poison water is instant death (along with falling out the course).
I entered that level with 50 Marios and finished on my final Mario.
3) That Pachinko-inspired special course on Isle Delfino. You got to reach eight red coins without falling out the course. Very time consuming. >.<;;
4) Corona Mountain's driving the boat through the lava section especially when blue coin hunting. The boat will sink if you crash it (lava instantly kills you too).
Oh here's one from Mario 64: That Red Switch course set high above the castle. Good luck getting all the red coins in one go.

fun fact: any level on mario kart wii sucks balls when you're online, probably because the multiplayer is absolutely broken.

Battle for Storm Hill in DKB. The level itself wasn't hard, keeping a good combo and holding onto the bananas for the upcoming boss fight was.
