There are a few aesthetic changes.
Black2's route 4 is now a city instead of a desert, and White2 is still a desert with a few inhabitants (another play on the technology vs. nature)
There's also one cave in the game which is a magma cave for Black2 and a frozen water one in white2 (or the other way around, I just know they're different between games).
There are also a few other things, but I tried to avoid them because of spoilers.
