Mariowiki says it's from Magma Castle, the last level.
Such a cool looking baddie, and he only got in one level

I'm guessing he's just using an emulator and setting the graphics plugin to dump the textures. That's how I've always done N64 textures anyway. I can give a more in depth explanation if you'd like.
