For an example, I currently have a model in the approval queue that is very strange-looking. It's unused in the game, and has a...unique texture layout. It's Knuckle's Studio from The Wind Waker. Because it looks so messed-up, I'm afraid that the approver might question its authenticity, or something to that effect.
In short, it would just be a nice way to communicate with the approvers, be it over a citation or any other concern or issues.
I'd like to think our staff would look into something before just blanket assuming it's broken or fake. This still isn't out of the question but I'm just having a hard time thinking of instances where it would really be necessary (especially at a volume to justify adding it). Others are welcome to chime in here as well of course.

Customs isn't really a genre though. Other is really for anything that doesn't fit into one of the other categories but not all customs will wind up there to begin with. Custom work based on existing work should go in the genre of the existing game. Completely custom work can either be placed in the genre that it was intended for (e.g. if you do 8-bit Mario inspired characters from another game, the platformer genre would likely be where you'd want to put them). Again, anyone else can offer feedback on this one as well.
