That is the item graphic place holder. It fills empty slots where items should be in the vram (until you get them).

Underground Caverns Miscellaneous Objects

FINALLY, this headache is DONE.
One fewer frame than I originally thought, but it was still tedious. >_<

If anyone wants to finish the remaining items from Symphony (armors, headgear, cloaks, accessories) please go right ahead. I'm tired of Symphony already. :p

To answer your question, Gizmonicgamer:

[Copy Pasted from my own little tutorial that I wrote for someone asking me the same thing]

Playstation ripping is a lot easier than one may initially think. There's a program you can pick up on Rom-hacking.net - it is called PSX-VRAM or just simply 'VRAM'. What it does is that it looks into save states of epsxe emulator save states. What you do is first:
1. Create a save state in the game you want to rip from.
2. Take your save state and change its file extension to .gz, an archive file.
3. Open the gz archive and drag and drop into the vram's open window.
4. The vram window will instantly display everything currently loaded into the Playstation's random access memory (will not always work on the first attempt, however, especially if it's your first time using it). The amount displayed varies from gaem to game. Sometimes, it will show every sprite that each character currently featured has to offer in the game. Sometimes it will show only a few poses or even only 1 at a time.

At first, it will look like a bunch of squashed pixel blobs.
You have to press 9 to view sprites or 0 to view the map tiles. Then with the W, A, S, and D keys move the main cursor (indicated by a large rectangular red outline) to view the sprites in their full size at the bottom of the vram window.

Now you will notice that either the sprites don't appear (if you pressed a number) or are black and white (if you did not). To find the correct colors for each sprite, you must use the arrow keys to move a red crosshair around. You will also notice a bunch of long colored strips located in certain areas throughout the vram area. These are the color palettes currently being used by the iso. Move the crosshairs over them until you find the correct colors for the sprites/tiles you want to rip.

Another thing you may notice, is that your crosshairs will jump several pixel rows at a time. This will make it appear that your color palette that you need to find isn't present in the vram. This is not so. Simply hold the shift key while hitting the arrow keys to shift your pixel over one pixel at a time. This will reveal MANY more color combinations.
